Hello, I know the importance of good wound pics, ie; before/after debridement, skin sub, etc...but what exactly should be in the photo?
1. a ruler? is this put near the wound or on the wound
2. name, date...is time necessary
3. location, stage
4. can this be on an old fashioned index card- IDK have the means to get wound software at present.
5. anything else

PS I am concerned that pics do not capture all. I cannot show a graft stuff in a wound, for ex. Would I take the pic and just document that too?

I would like to mention that I will put serial # of grafts on the index card too
